We had a Covid vaccine Yellow Card monitor in the UK. Not sure of participant numbers but such a prompted self reporting trial should have eliminated the "passive" from the surveillance system and hence most underreporting, excluding of course incapacitation and death. Such trials could raise reporting by 100X. The MHRA advise that 1 in 10 people experienced an adverse event in the Pfizer trials, similar to the extreme numbers here.

Here's a picture of a Pfizer vial that helps explain why the control codes like PAA156571 are mistakenly entered in the vax lot field by people: https://cloudfront-ap-southeast-2.images.arcpublishing.com/nzme/U6F6CCEEEDGGUEWWUBKIZLITW4.jpg

... like the two in Eagle's chart above that he highlights for us with the question mark. Pfizer's vials seem designed to generate errors with plausible deniability, font is odd (hard to read for certain characters/letters), the PAA code has the same color scheme as the words LOT/EXP: and as a result, the entry in VAERS is often just the PAA code alone from what I've seen.

I looked through all of those above with the question mark and they do all have the corresponding correct counterpart in the list just above them.

Patients must be arriving at Emergency with the codes like that on their vax card from the location where they were jabbed.

Update of lot code cleaning coming soon, I'm behind schedule.

This is an instructional .gov PDF telling them quote: "Do NOT use the PAA number in the lot number field." It's confusing even in their instructions, UoU means Unit of Use and they say

"Pfizer: the UoU lot number is located in the LOT/EXP field."

No, it's located immediately ABOVE the words LOT/EXP, not IN it. The LOT/EXP field is BELOW the words LOT/EXP which by the way should be in it instead for clarity to avoid errors. And to add to that, their image example has no content for Lot or Expiry in that field, it is left blank.

But such a remarkable level of competence is consistent with the FDA's friendly reminder to tell the doctor holding the needle if your 5-year old son is pregnant: https://archive.ph/rxD3j

Thank you for this.

Jessica was indeed right that most the deaths were in the old.

But there are substantial numbers of young deaths reported there too.

Yes, it was a nice educated guess, for the general population I created the dashboard.
